Hospice Toronto (formerly Trinity Home Hospice) provides continuity of care, through volunteer care teams, to people living with life-threatening illnesses.

Hospice Toronto helps those who are terminally ill die in comfortable environments with peace and dignity. Hospice Toronto services complement client and community resources through the provision of integrated practical care and emotional support to people with life-threatening illnesses, as well as to their families and friends.
